The Internal conflict is in Before when Claire has to go through losing (literally) her child, Gabriel when Jonas left and took him. The external conflict is during Between when Claire must go through many hardships when she is training to climb the mountain to see Gabriel again.

What happened at the end of The Giver? At the end of The Giver, Jonas and Gabe head down through the snow to a place where there is music. Downward, downward, faster and faster. Suddenly he was aware with certainty and joy that below, ahead, they were waiting for him; and they were waiting, too, for the baby.
